For decades, dental insurance has played a major role in helping patients access care and helping practices maintain a steady flow of patients. But as the economics of dentistry continue to change, more practice owners are asking an important question:
Are dental practices becoming too dependent on insurance?
The Pressure Is Growing
The cost of operating a dental practice continues to rise. Staffing, supplies, laboratory fees, technology, rent, and other overhead expenses have all placed additional pressure on practice owners.
At the same time, many dentists feel that insurance reimbursements have not kept pace with those increasing costs.
The result can put practices in a difficult position: provide care at today's costs while being reimbursed according to fee schedules that may not reflect today's economics.
And reimbursement is only part of the conversation.
Insurance participation can also bring additional administrative responsibilities, including claim submissions, pre-authorizations, documentation requirements, eligibility verification, appeals, and collections.
For some practices, managing insurance has become a significant part of running the business.
Is Fee-for-Service the Answer?
Some dentists are responding by reducing their participation with PPO plans or transitioning toward a more fee-for-service model.
The potential advantages are attractive: greater control over fees, fewer insurance restrictions, less administrative burden, and potentially more freedom when discussing treatment with patients.
But leaving insurance networks isn't necessarily easy.
Patients may have become accustomed to using their benefits, and practices must consider how changes in participation could affect patient retention, new-patient acquisition, and case acceptance.
For many offices, the answer may not be completely insurance-based or completely fee-for-service.
A hybrid approach may ultimately provide the right balance.
The Bigger Question
Every dental practice has a different patient population, overhead structure, market, and business model. There may not be one strategy that works for everyone.
But as the economics of dentistry evolve, understanding the true impact of insurance participation is becoming increasingly important.
Practice owners should know what each insurance plan actually contributes to their business—not simply how many patients it brings through the door.
